Submit your form by:

• Mailing it or dropping it off to the parish office at: 826 W Central Ave, Davidsonville, MD 21035
• Placing the form in a sealed envelope and drop it in the collection basket during Mass.
• Dropping it in one of the poor boxes when you come to Mass on the weekend.
• Placing it in the drop box outside of the church by the far left double doors.
